INSURANCE INFORMATION

Medical Services:

We participate in most insurance plans. 

Medicare assignment is accepted. We bill Medicare electronically for you.

Although we may participate in your insurance plan, your insurance may not cover every service provided in this office. It is your responsibility to understand the limits of your insurance before seeking care.

The philosophy of our office is to provide the highest level of individualized service. However, we cannot function as your insurance company and do not have the ability to explain details of YOUR INSURANCE POLICY. Please read your insurance policy or contact your insurance carrier if you are not sure of the limits of your coverage. 

If your insurance plan requires a REFERRAL from your PRIMARY CARE PHYSICIAN, it your responsibility to bring us a VALID, UNEXPIRED REFERRAL.

We accept Suffolk County Vytra and the Max access plan. We are not currently accepting the Vytra HMO. If your Vytra card lists a radiologist, then you are in the HMO. 

If you need a referral for your visit, please insure that you obtain it prior to your visit.
