Driving directions:

From Sunrise Highway:

Take Route 231 north about 3 miles. Go under the LIRR tracks and make a left on Nicolls Road (Gulf gas station on the corner). We are the first building on the left, 590 Nicolls Road.

From The Long Island Expressway.

Take exit 51 (Deer Park Avenue) south about 2 1/2 miles to Nicolls Road (Gulf  gas station on the corner). Make a right turn on Nicolls Road and a left turn into the the first building on the left, 590 Nicolls Road.

To Holbrook Office       Phone:  631.285-7311

From Sunrise Highway:

Take Waverly Ave (CR19) North and continue straight onto Patchogue Holbrook Road  to Union Avenue. Make a left on Union Avenue and a right turn into our parking lot at 233 Union Avenue, Holbrook Medical Center..

From The Long Island Expressway:

Take exit 61 (Patchogue-Holbrook Road or CR19) South one block to Smith Avenue (Exxon Station). Make a gentle right turn on Smith Avenue and take to the end which is Union Avenue. Make a right on Union Avenue and another quick right turn into our parking lot at 233 Union Avenue, Holbrook Medical Center..
